Allied health support means hands-on help that is linked to your health - so let’s have a chat. We focus on routines that work in real life, not just in theory. We include support after hospital stays and during early recovery.

 

Your clinician’s plan guides the work - we keep each step simple and doable. You can start small and add items when you’re ready. Small wins build confidence and help each week feel easier.

The First Weeks After Discharge

Coming home can feel big. We line up small steps that make each day easier. We can pick up scripts, stock the fridge, and help you get to follow-ups.

  • We check your equipment list and set things up safely.

  • We plan the first week so rest and care both happen.

  • We confirm who to call if something changes overnight.

  • With your okay, we update family members on anything important.

Appointments, Plans And Life Happening

We plan routes, set pick-ups, and confirm returns to get you back home safely. Your worker can stay with you and help at each stop if you want that.


We check access and parking before the day and can help with forms, sign-ins, and payments if you need us to.

Meals, Hygiene, And Light Housework

We agree on plans in writing so everyone knows what to do and what’s happening next. This can include morning routines, meal prep, light cleaning, and notes for next time. Ask us to print the plan for you to stick up on your fridge.


We tidy as we go and make notes of any new needs we see. Bigger jobs are broken into short, simple steps. We’ll fit visits around meals, meds, and rest times that are comfortable for you.

Following Therapy Plans At Home

 

We follow the plan set by your physio, OT, nurse, or doctor. You get steady practice in real life settings, at a pace that suits you.


We read the letters and confirm any precautions first so everyone stays safe. If something is not helping, we bring it up early so we can come up with a plan quickly.


We share updates in a format your clinician can use.

Basic Wound Care At Home

​

We can change simple dressings if your clinician has directed this. If a nurse is required, we’ll say so straight away and help to get one booked. 

​

Steps we follow include:

​

  • setting a clean area and gathering supplies before we start.

  • recording the time, site, and any signs to watch.

  • we follow your product list and note batch numbers if needed.

  • we show you what to monitor between visits.

Physio And OT Home Plans

This can include gait drills, hand exercises, and fine-motor tasks. We help you to practise the exercises and steps in your home plan. â€‹

 

  • Set small goals for practice between sessions.

  • Timers and counters keep the pace safe and steady.

  • If pain rises, we swap tasks or reduce sets.

  • We share progress notes with your clinician on request.

Using Equipment Safely

 

Think shower chairs, raised seats, pressure cushions, or bed poles. We set up the items your clinician recommends and show you how to use them safely. We do this by:

​

  • checking height, reach, and clearance in each space.

  • labelling settings so anyone can reset them.

  • small layout changes that can make moving safer.

  • keeping a list of items to review with your therapist.

Medication Prompts And Health Notes

We can remind you when to take medicines and we take notes on how you’re going. We keep simple logs you can share with your GP or therapist.


Records stay short, clear, and easy to bring to appointments. We note how you feel and any effects you report.

Everyday Jobs We Can Handle For You

 

Your goals guide the plan - we just help to keep the steps clear. Tasks change with your needs and the clinician’s advice.


We’ll agree on the list each week and update it as things shift. If another provider is a better option for you, we will let you know.

 

  • Dressing changes and basic wound checks (as directed).

  • Physio or OT home plan practice.

  • Meal prep, simple shops, and kitchen set-up.

  • Transport to clinics and back home again.

  • Equipment set-up and safe use tips.

Funding And Referrals

Funding can come through NDIS, TAC, or WorkCover, depending on your case. We work within current rules and confirm details in writing before visits.


Some items need pre-approval, and we explain that at the start. You’ll see all rates and time before anything is booked.


Invoices match the wording used by your funder so that it is easy to understand and adjust things for you if needed. In our feedback, we show what was done, when, and by whom.

From First Call To First Visit

A short call gets things moving. We note your goals, suburb, and best times. We then match a worker, confirm the plan, and set your first visit.

You can start by phone or email, whichever you like better. We send the plan and the worker’s name in writing.

You can change times by phone or email at any stage.

​

What To Expect On The Day

​

We arrive within the agreed window and say hello to you by name. We review any risks and set up the space for what we are working on that day.
